Observation

During the Complaint Inspection and interviews, it was found that a child was having regular health symtoms involving breathing or lip turning blue as symptoms, provider was not able to get the parents to update health plan agreement or ICP, nor report to department concern as mandated reporter of not having a health plan in place for child's on-going health needs or symptoms.
